The patient consulted at Al Salam Health with the following concerns:

• Biting and Speech Issues:

  • Difficulty in biting and chewing food properly
  • Speech difficulty

• Appearance Concerns:

  • Unhappy with the way the teeth look
  • Felt shy in social situations due to irregular teeth

• Oral Health Concerns:

  • Gum inflammation and recession
  • Plaque buildup in crowded teeth areas due to difficulty in cleaning

• Support and Joint Concerns:

  • Reduced cortical bone support around the teeth
  • Jaw joint discomfort (TMJ)

Before Treatment (Initial Assessment)

An initial clinical examination was carried out to evaluate the affected teeth and surrounding structures.

Assessment Included:

  • Clinical dental examination
  • Intraoral evaluation
  • Extraoral (facial) assessment

Consultation & Advanced Diagnostics

During the consultation, the patient’s medical history was analyzed, and the dental condition and treatment approach were discussed.

Diagnostics Used:

  • OPG and lateral cephalogram X-ray — to assess the general bone health and root conditions
  • Clinical intraoral photographs
  • Upper and lower study models (alginate impressions)

  • Mesofacial pattern
  • Incompetent lips
  • Convex facial profile

  • Angle’s Class I malocclusion
  • Severe crowding in upper and lower arches
  • Labially positioned canine (#6)
  • Posterior crossbite on the left side
  • Midline spacing (diastema) in upper teeth
  • Rotation of teeth (#20, 22, 27, 29)

Treatment Procedure

  • Fixed orthodontic treatment using PEA appliance
  • Non-extraction approach followed initially, with review for extraction if required
  • Upper and lower braces placed

Treatment Considerations

  • Severe crowding may require tooth removal, but the patient preferred to avoid extraction
  • Alignment without extraction can lead to forward movement of teeth
  • The patient has a good soft tissue drape which can mask the forward movement of teeth
  • Controlled tooth movement resulted in proper alignment with minimal forward movement of teeth

Treatment Duration:

  • Approximately 18 months

After Treatment (The Result)

After treatment, the patient showed:

  • Teeth aligned in both upper and lower arches
  • Bite corrected with proper contact between teeth
  • Noticeable improvement in smile appearance
